Information and Consent
for Event/Activity form
Parental permission is required for
adventurous activities and for activities that are undertaken out
of the usual meeting times and away from the meeting place.

If undertaking multiple activities, it is acceptable to list
them all on one form.
Making your own form
Leaders must not make their own
forms.
It would be difficult for Girlguiding UK to support
you if an accident or incident should occur and the official
forms were not being used.
